Each student with a disability has needs with a differing depth and scope for accommodations. In response to the need for accommodations the university requires students to provide complete and current documentation of their disability. This section contains information necessary regarding the specific documentation needed by The Office for Students with Disabilities to evaluate your request for accommodations, auxiliary aides and services. The format of the needed documentation was developed using the guidelines of the Association on Higher Education and Disabilities (AHEAD). The intent of these guidelines is to provide the student, the diagnostician, parents and service providers with a common set of understandings of the components of documentation, which are necessary to establish eligibility for services. Evaluation: The documentation must be provided by a professional who has a specialization related to the disability. Examples: a school psychologist or learning disabilities diagnostician must verify a learning disability; a psychiatrist, psychologist or clinical social worker must verify a psychological disability; an audiologist must verify a hearing impairment, ect. Letters stating that the student has a disability but which do not provide any diagnosis or evidence of a history of accommodations are NOT acceptable.
There are four areas of importance:
RECENCY OF DOCUMENTATION
Testing must be current. As a general guideline the testing must have been conducted with-in the past three years. The provisions of all reasonable accommodations must be based upon the assessment of the current impact of the student’s disability on his/her academic performance.
The documentation must include clear and specific evidence of the disability. The following MUST be included: summary of procedures and test instruments used for assessment; summary of assessment results including test results and evaluation, name and title of evaluator, and dates of testing; recommendation for appropriate and reasonable accommodations; medical history if student is on medication, including impact of current medication on students academic performance. Individual education plans (IEP) and section 504 plans must be accompanied by supporting documentation to establish the rational for accommodations. The documentation must be submitted on letterhead of the appropriate professional diagnosing the disability.
A complete explanation of the appropriate accommodation. The explanation must show a clear relationship between the student’s disability and the accommodation. The OSD will review the documentation and will determine the appropriate academic accommodation for each student based on the student’s documentation. The appropriate Dean will make the final determination for students who are requesting a substitution of a degree or course requirement.
REFERRALS FOR EVALUATION
Siena Heights University does not provide evaluation or assessment services. The OSD will provide information to students interested in obtaining testing, evaluation or assessment services.
